As far as I'm aware this hasn't come up at all, but the other day I
assured someone under their non-married name, because they only just got
married 1 week ago and as far as I know we have no official policy for
change of name requests. The only other similar thing that has occurred
is when people enter in the wrong name/date of birth etc, and request
fixes etc.
