If that was the only reason, then it would be reasonable to add a tickbox to the Assurance form that said "over 18?" and to discard the DoB on reaching 18. Or somesuch.

Also, we should think carefully if we need the under 18 / over 18 distinction, and it's usage is just because the DoB is recorded. In the absence of the DoB what problems would there be?

(It isn't the only reason, Duane identified the partial uniqueness issue, but again there are solutions to that elsewhere.)
