I think we have a rather simple mechanism for that already:
The experienced assurer should contact 
support AT cacert.org
 and provide all 
details about the problem, the support team will care about it then, and 
revoke the assurance and to tell the new assurer how he should handle it in 
the future.
I don´t think that we need any more complex mechanisms there.
